The Medication Administration Record (also called the MAR or Kardex) contains all the Regular Scheduled
Medications, PRN Medications and Treatments that the specific resident's physician has ordered for
the Nursing Staff to administer to that resident at one point during a specific time period. This
form is to be used by the Nursing Staff as a method of noting that the specific order has been
administered to the resident at the correct time, by the proper route and for the proper indication.
The information used on the Medication Administration Record is taken directly from the Doctor's Order
Form and any Intervening Order Sheets used. Times of administration, original order dates, and helpful
hints on drug administration are also included.
